激越型うつ病にmirtazapineが奏効した3例―薬理学的疾患分類の可能性を鑑みて

　うつ病を巡る議論は精神科医療の枠にとどまらず，社会を巻き込んで大きな問題となっている。臨床的特徴をもとに非定型うつ病を提唱したKleinらは薬理学的疾病分類を目指したが，これらは操作的診断において平板化しているうつ病の薬物選択に対する警鐘でもあり現在でもなお有用な方向性であると考えられる。今回我々はmirtazapineが奏効した3例の激越型うつ病と考えられる症例報告を通じて，薬剤反応性を基板にしたうつ病細分化の可能性について論じる。ある一面ではあるが，このような方向性が肥大するうつ病の疾患喧伝や安直な操作的診断基準の適応に対する一つの回答になりうるのではないかと考えている。
