緊張病性昏迷に対してolanzapine速効型筋注製剤が有効であった1例

抄録

　統合失調感情障害での緊張病性昏迷にolanzapine速効型筋注製剤(OLZ筋注)が有効であった1例を経験した。症例は49歳，男性。人格解体や性的逸脱行動などに対し，haloperidol 39mg，risperidone 12mg，levomepromazine 400mgなどを含む多剤大量処方が行われていた。緊張病性昏迷に対しOLZ筋注10mgを施行したところ，翌日には昏迷は消失した。Olanzapine 20mg経口剤に切替えたが，精神症状の悪化は認めず，自発性が改善した。OLZ筋注の薬物動態がカタトニアの早期改善に有効であったと考えられた。
