うつ病の発症は予防できるか，減らすことができるのか―認知機能障害とレジリエンスの視点からのうつ病予防の可能性

はじめに

　厚生労働省の報告によると，精神疾患の患者数は増加の一途を辿り，なかでも医療機関を受診したうつ病患者数は2011年に70.8万人と12年間で2.9倍に増加した。飛鳥井³⁾によれば，自殺死亡者数3.1万人の約9割が精神疾患に罹患していたという報告があり，自殺に深く関係しているうつ病対策は喫緊の課題であると言える。うつ病を筆頭に増加する精神疾患を背景として，2013年度より精神疾患が5疾病5事業として医療計画に追加され，増え続けるうつ病に対して，国はうつ病の根治的治療の確立を目標としている。その中には，広い意味でうつ病の発症予防も含まれていると理解できる。

　疾病予防には，疾病を未然に防ぐ1次予防から，重症化を防ぐために早期発見して早期に治療する2次予防，そして，再発を防ぐ3次予防の3段階がある。同様の分類には，Institute of Medicine(1994)によって提示された精神障害に対する介入スペクトラムによる予防介入があり，大集団を対象とする普遍的(universal)介入による予防，リスクのある人を対象とする選択的(selective)介入による予防，病気の初期兆候がみられる人への指標的(indicated)介入による予防の3段階に分類されている。本邦では，いずれの段階の予防もメンタルヘルスとして実施されており，この3段階の予防によってうつ病の患者数が減っていくものと考えられる。
