睡眠の改善によるうつ病予防は可能か

はじめに

 不眠や過眠などの睡眠障害は,うつ病においては初期から認めるほぼ必発の症状であり4,50),治療経過をみる上で重要な指標となる。さらに,不眠や過眠は,診断の上で重要な症状の1つとされ,Diagnostic and Statistical Manual of Mental Disorders, 5th ed.(DSM-5)などの操作的診断システムにおいて,うつ病診断基準の1項目として取り上げられてきた4)。一方,疫学研究においては,長期的にみると不眠の既往のあるものはうつ病に罹患しやすいなど,睡眠の問題がうつ病の危険因子になり得ることも分かってきた51)。治療的側面については,古くから持続睡眠療法や断眠療法など適切な睡眠操作がうつ病の治療に応用できることが知られており9),最近では,うつ病に伴う不眠に対して,睡眠薬あるいは不眠症に対する認知行動療法(Cognitive Behavioral Therapy for Insomnia;CBT-I)などで積極的に治療すると抑うつ症状自体が改善することが報告されている17,33)

 本稿では,まずこれまでの臨床研究および疫学研究を展望し,うつ病における睡眠の問題,うつ病治療における不眠治療の意義,うつ病の危険因子としての不眠について解説する。その上で,睡眠に対する働きかけによるうつ病予防について,最近始まったいくつかの試みを紹介し,その可能性について考える。
