うつ病の発症予防とレジリエンス

“Resilience”は9世紀の西欧で「弾力」や「反発力」を意味する物理学的用語であった。それが20世紀の70年代に児童精神科領域で，逆境の中で生育し立派な大人に成長した子どもを形容する際に用いられ(“resilient”)，80年代から精神疾患に対する防御因子と抵抗力を意味する概念として成人の精神医学に導入され始めた34)。特に90年代には心的外傷後ストレス障害(PTSD)について危険因子とともに「防御因子」が注目され，21世紀に入ってレジリエンス研究はその他の精神疾患へと急速に拡大している。“Resilience”の決まった訳語はまだないが，筆者らは，発病「脆弱性」に対置される健康時の発病「抵抗力」と，発病後の健康「回復力」という二面を持つ概念として理解し「疾病抵抗性(あるいは抗病力)」の訳語を用いた40)。近年では，「困難な状況や心理的ストレスを経験しても速やかに元の健康状態に戻る能力，精神的安定を維持する能力」28)と捉えられている。いずれにしても，近年のレジリエンス研究の活性化を，これまでの精神医学が疫学研究においては“Risk Factor”(危険因子)の発見に，生物学的研究においては“Vulnerability”(脆弱性)の解明に偏りすぎていたことへの反動と見ることもできよう。レジリエンス研究のさらなる発展により，健康時の発病抵抗力と発病後の健康回復力の解明が進み，精神疾患の治療論と回復論の発展に寄与するとともに，本稿のテーマである健康維持論，発病・再発予防論へ有用な知見を提供すると期待される。
