はじめに
 嗜銀顆粒(argyrophilic grain)とはタウ蛋白が神経細胞の突起に蓄積した病変である。嗜銀顆粒の出現した状態は嗜銀顆粒病(argyrophilic grain disease;AGD)と呼ばれる。最初に報告されたAGDの臨床像は認知症状態である5)。しかしその後,初老期以降のいくつかの精神科状態像と関連している可能性や,AGDが他の変性疾患に合併して出現することで臨床特徴を修飾し得ることが明らかとなり,さらに他の変性疾患と病態レベルで関連する可能性も示唆されている。本稿ではAGDにおける認知機能障害,精神症状,生活障害を理解する上で重要と思われる基礎と臨床の知見を整理する。
