ラメルテオンが悪夢・不眠に奏功した特発性レム睡眠行動障害の1例

　近年，特発性レム睡眠行動障害（REM sleep behavior disorder；RBD）は，α-シヌクレイノパチーなどの神経変性疾患の前駆症状である可能性が指摘されている。特発性RBDでは動作緩慢・認知機能低下などが出現し得ることから，クロナゼパムを第1選択薬とした薬物治療においては副作用に注意すべきである。ラメルテオンは鎮静・筋弛緩作用や認知機能低下へのリスクが比較的少ない薬剤であり，神経変性疾患を考慮した薬物治療として有用な可能性がある。しかし，ラメルテオンの特発性RBDに対する有効例の報告はほとんどない。我々は，特発性RBDの悪夢・不眠に対しラメルテオンが奏功した症例を経験したため，文献的考察を加えて報告する。
