双極性障害における閾値下症状の考え方と早期介入

双極性障害は人口の約1％が罹患する，慢性，再発性の気分障害である。好発年齢は20歳前後で，罹病期間は患者の人生の長きにわたる21)。またGlobal Burden of Disease Study 2013によると躁うつ病のyears lived with disabilityは全301の急性・慢性疾患中，第17位にランクされている12)。このように社会的影響も非常に大きく，早期介入により顕在発症の予防が可能となることが望まれる疾患の一つである。
近年，統合失調症に対するAt Risk Mental State(ARMS)研究が活発に行われており，前駆症状の評価ツールの開発，またそれらを通じたフォローアップ研究，予防の取り組みの成果が得られている。双極性障害に対する早期介入の研究もこれに影響を受けてか，近年盛んになってきている。
本稿では，双極性障害の早期介入に関する種々のエビデンスをレビューする。はじめに発症のリスクファクターという観点での研究を紹介し，次に双極性障害を気分障害の連続性，すなわちスペクトラムとして捉えようとするアプローチについて紹介する。また，日本における双極性障害の考え方，特に下田の執着気質について注目した。さらに閾値下の前駆症状を捉えるためのスケール(BPSS-P)を紹介し，最後に早期介入の試みについてのエビデンスをまとめた。
