不安症への早期介入

統合失調症やうつ病，双極性障害などの内因性精神疾患は，当初不安症状にて発症しその後上記診断に移行することが多く，早期の不安症状のコントロールがより重篤な精神疾患の発症を予防する可能性がある。しかし，不安症は専門医でなければ見逃されがちな疾患であり，患者自身も医療機関へ相談することが少ないためか，組織的な早期介入はこれまでに行われていない。
本稿では，不安症を疫学的な観点から概観し，その上で早期介入の可能性について若干の考察を加えたい。DSM-5では，従来の不安障害のうち，強迫症および心的外傷後ストレス障害などのストレス因関連障害がそれぞれ別カテゴリーとなり，パニック症，全般不安症，広場恐怖症，社交不安症，限定性恐怖症，そして新たに分離不安症と選択性緘黙が不安症のカテゴリーに含まれることになった。DSM-5を診断カテゴリーとして用いた大規模な疫学調査はまだ行われていないが，DSM-5の不安症に含まれる疾患の概観を行うこととする。
