ADHDの衝動性と関連する疾患

注意欠如・多動症(attention-deficit/hyperactivity disorder；ADHD)は，不注意，多動性，衝動性といった行動上の特性によって特徴付けられる神経発達症である。さまざまな生物学的要因を基盤に，養育に関連した心理的要因や環境要因，さらに行動統制を要求される現在の生活環境などが複雑に絡み合って症状が惹起あるいは悪循環するといわれる。ADHDの中心症状である衝動性は，さまざまな精神疾患でも認められ，鑑別が求められる。また，それらの精神疾患はADHDの併存症として捉えられる場合もある。本稿では，ADHDの衝動性に関して遂行機能障害や報酬系の機能障害との関連などについて論じ，続いて衝動性が関与する疾患を挙げながらADHDとの関係または併存に伴う影響などについて概説する。
