反復経頭蓋磁気刺激(rTMS)による治療抵抗性うつ病の治療と機能的結合の変化

抄録 うつ病は,前頭前野と辺縁系領域の活動の機能的不均衡が病態に関与している。左前頭前野への高頻度rTMSは,前頭前野の低活動,辺縁系の過活動を是正することが知られている。一方,神経ネットワークへの作用機序は明らかではない。本研究は,治療抵抗性うつ病に対して高頻度rTMSを施行し,その前後で安静時脳波を高密度脳波計で計測した。また,安静時脳波はsLORETAを用いて解析し,rTMSによる機能的結合の変化を調べた。結果は,左側背外側前頭前野と辺縁領域間のmiddle beta帯域を同調させ,GABA抑制系回路の修飾を行う可能性が考えられた。
