認知機能障害の概念—社会機能向上に向けた鍵として

はじめに
　古くから「ヒトとは何か」との問いに対して，認知・感情・意欲の3つの機能の重要性が指摘され，盛んに研究がなされてきた。情動については，辺縁系を中心とした心理学的・生理学的研究で，その機能について，種々の知見が得られている。一方で，注意や記憶，遂行機能などの認知機能については，その詳細は十分に明らかではなかった。
　1990年代以降の神経科学の進展に伴い，遺伝子，分子，脳画像，計算理論などの複合領域で脳機能の解明が進み，沈黙野と呼ばれていた前頭前野の機能が解明されるにつれ，認知機能の詳細な神経メカニズムが徐々に明らかになっている。
　一方，精神医学においては，認知機能とその障害については，古くBleulerの時代からその重要性は認識されていたが，治療対象として取り上げられることは，最近までなかった。しかし2000年代に入り，第二世代の抗精神病薬が普及するにつれ，統合失調症において，副作用としての認知機能への影響のみならず，認知機能の改善効果が注目されるようになった。同時に，治療目標が，単に精神症状の改善にとどまらず，患者個人が主観的満足度の高い生活をいかに送るかという「リカバリー」へと転換し，これを実現するために，日常生活の生活しづらさをいかに解消するか，つまり社会機能の向上をいかに図るかが，注目され始めた。近年，主要な精神疾患において，社会機能の低下には認知機能障害が強く影響していることが明らかにされており，認知機能障害をいかに評価し，治療するかがきわめて重要な課題となっている。
　このように認知機能に関する意識の高まりと相まって，精神医学における認知機能障害の評価と治療の重要性がここ20年間で飛躍的に高まっているが，ひと口に認知機能障害といっても，その内容は多岐にわたる。
　本稿では，認知機能障害が主症状の一つとして位置付けられ，その評価と治療法について，盛んに研究されている統合失調症を中心に取り上げ，1)認知機能とは，2)認知機能障害とその種類，3)認知機能障害の評価，4)認知機能障害の治療，5)情動と認知機能障害の関係性について，概説する。重要なことは，患者の社会機能を改善・向上する上で，どのような認知機能障害が問題となっているのか，どの程度の重症度で，どのような治療を行う必要があるのか，医療者間で同じ認識を共有することにあると思われる。
