アルコール離脱せん妄

はじめに
　2013年に行われた疫学調査によれば，アルコール依存症疑いの患者は全国に108万人いると推定されている。アルコールはさまざまな身体疾患を引き起こし，また転倒，転落などの事故のリスクも高いため，救急受診につながりやすい。そのため，アルコール離脱はさまざまな科で遭遇することの多い病態である。
　アルコール離脱によるせん妄は振戦せん妄とも呼ばれ，アルコール離脱てんかんと並んでアルコール離脱の最も重症な形態である。振戦せん妄は，救急医療や集中治療室での対応困難を引き起こすだけではなく，致死的となる可能性もあるため，的確な診断とマネジメントの必要性は高い。本稿では，アルコール離脱せん妄の病態，治療についての概要を述べたい。
