高齢初発てんかん

はじめに
てんかんは小児の病気と誤解されることが多いが，数々の疫学研究により発症率は年齢とともに上昇し，65歳以上の高齢者で最も高くなることが示されている。てんかんは脳に生じるあらゆる疾患を背景として起こるため，脳卒中や神経変性疾患の有病率が高い高齢者でてんかんが増えるのは，実は自然なことと言える。今後の高齢化社会では，その患者数はさらに増加すると予想される。高齢発症のてんかんは，若年者と臨床像や検査所見が異なる上に，鑑別すべき疾患も多岐にわたるために診断が困難であることが多く，過少診断または過剰診断となることが少なくない。また，高齢者は薬物代謝能が低下しているため，治療においては副作用に注意し，薬剤選択の際は併存疾患やポリファーマシーを考慮する必要がある。一方で，高齢者てんかんは薬剤反応性が比較的良好であることが知られており，適切な診断・治療により患者のQOLに大きく貢献することができる。高齢者のてんかんに特徴的な臨床像も知られており，それらが診断のヒントになることもある。本稿では，高齢初発てんかんのマネジメントに必要な基礎知識を提示する。
