双極Ⅱ型障害について—双極Ⅰ型障害の軽症型との理解ではなく

はじめに
双極Ⅱ型障害とは一般の精神科医にとってどんなイメージの疾患であろうか。明らかな躁病相とうつ病相が交代する双極Ⅰ型障害は，古典的な躁うつ病の理解が当てはまりやすい。精神科病院に勤めていると，典型的な躁状態の患者が興奮して搬送されてきて，声高にまとまりなく誇大的な内容を話し続けるといったことから，比較的容易に双極Ⅰ型障害の診断がつく。患者に振り回されて周囲の人たちも疲弊しており，「何とか入院をお願いします」と懇願することになり，医療保護入院での治療が開始されるであろう。服薬可能な場合には，治療ガイドラインにのっとり，リチウムまたはバルプロ酸を主剤として，必要に応じて非定型抗精神病薬を使用して，静穏化をはかるという治療コースもおおむね敷かれている。
それに比べて，双極Ⅱ型障害は確固たるイメージを持ちにくく，全体的にぼやけた印象があるのではなかろうか。一般的には，双極Ⅰ型障害の軽症型として捉えられて，双極Ⅰ型障害が激しい躁状態を呈するのに比べて，双極Ⅱ型障害の場合には，多弁で活動性が上昇するが，易怒性は比較的抑えられており，逸脱した行為は少ないなどのかなり恣意的な判断で，軽躁状態とみなされ，診断としては「強いて言えばⅡ型になるだろう」として，双極Ⅰ型障害に準じた治療を開始するということが日常的に行われているように思われる。実際に，日本の双極性障害治療ガイドラインに目を通しても，双極Ⅱ型障害についてほとんど紙面が割かれていない49)。
そのため，双極Ⅱ型障害は精神科医の誰もが知る疾患でありながら，漠然として捉えどころがない。双極Ⅰ型障害という強烈な印象を示す疾患に隠れて，どこか日陰の存在である。古くはDunnerらによって，反復性の抑うつ状態を繰り返していた入院中の患者の多くで，以前に入院する必要がない程度の軽い躁病エピソードがあったことが見出された24)。家族歴から双極Ⅰ型障害と同様に，その病態の遺伝的な要因も強いことが描出されて，双極Ⅱ型障害と名付けられた。それから20年近く経過して，DSM-Ⅳの中に，抑うつエピソードと軽躁病エピソードの既往の存在する場合に，双極Ⅱ型障害(軽躁病エピソードを伴う反復性大うつ病エピソード)との診断名が正式に記載された1)。DSM-Ⅳにおいて，躁病エピソードと軽躁病エピソードはその持続期間と重症度において区分されていた。持続期間は躁病エピソードの場合は1週間以上であり，軽躁病エピソードでは4日以上となっていた。重症度については，躁病エピソードでは「職業的機能や日常の社会活動または他者との人間関係に著しい障害を起こすほど，または自己または他者を傷つけるのを防ぐため入院が必要であるほど重篤であるか，または精神病性の特徴が存在する」とあり，軽躁病エピソードでは「社会的または職業的機能に著しい障害を起こすほど，または入院を必要とするほど重篤でなく，精神病性の特徴は存在しない」との説明であった。つまりは，重症度に関しては入院を必要とするかどうかと，精神病症状の有無を一つの目安としていた。
それらの記載からは，双極Ⅱ型障害は，躁的な状態の持続期間が短く，病状も比較的軽いという説明であり，双極Ⅰ型障害の軽症型との認識しか残らない。このような症状の軽重で，疾患を分類する場合は，軽症型は重症型が除外された場合にのみ該当することとなり，しばしば重症型にその疾患の定型的な病状を有する患者群が集中して，軽症型として非定型的な患者が拾われることになる。必然的に，重症型の診断の信頼性や妥当性は高くなるが，軽症型の診断の信頼性や妥当性は低く，軽症型の診断基準が疑問視されることとなる。DSM-Ⅳにおけるアルコール依存症とアルコール乱用ではまさにこのような上下関係にあり，依存症が除外された場合においてのみ，飲酒によって社会的な問題を引き起こしたときは乱用の診断が適用された。その結果，依存症診断の信頼性や妥当性は高かったが，乱用の診断基準は問題視され，DSM-5においてはアルコール使用障害として一連の疾患としてまとめられた32,65)。
同様の理屈から，DSM-Ⅳでの双極Ⅱ型障害の診断についても，DSM-5において双極スペクトラム症といったような名称で，一連の疾患として双極Ⅰ型障害とともに一括りにされてもよかったはずであった。しかし，双極Ⅱ型障害は独立した診断として残った2)。双極Ⅱ型障害や軽躁病エピソードにおける診断の信頼性に関して，確かに論争が続いている。ただし，構造化面接を用いた双極Ⅱ型障害の診断の信頼性は，双極Ⅰ型障害や大うつ病と同程度に高く，特異性の高い疾患であるとの報告がある55)。DSM-Ⅳに基づいた構造化面接でも，2つの面接法の診断一致率が双極Ⅰ型障害，双極Ⅱ型障害ともに高く，診断妥当性が示されている40)。双極Ⅱ型障害が特有の病態や病状経過を呈する疾患であるとの観察研究の積み重ねがあり，最近では，双極Ⅰ型障害とは異なる薬物治療の適応についてのエビデンスが集積されてきている実状がある。今回，主に双極Ⅰ型障害との異同を対比させることで，双極Ⅱ型障害に固有の特徴を浮き彫りにしていく。
