がん患者の不安と抑うつ

はじめに
　がんは日本人の死因の第1位を占め，毎年30万人以上の人ががんで亡くなっている。しかも，この数は高齢化とともに年々増加し続けており，2015年のがん死亡数は1985年の約2倍となっている¹⁹⁾。このようなことから，がんは，実臨床においては致死的ではなくなりつつあるものの，一般にみれば未だに死を意識せざるを得ない病気の代表と考えられている。また，がん患者は病気の経過中に，診断から始まって，進行・再発や積極的抗がん治療の中止など，何回にもわたって告知を受けることがあり，さらにさまざまな要因が加わってほとんどの患者は，程度の差はあれ不安や抑うつを来す¹²⁾。Mitchellら²⁸⁾は健常者を基準にして，がんサバイバーの不安の長期経過について調べた10編の研究および抑うつの長期経過について調べた16編の研究を用いて，それぞれの相対危険率を求めると，不安は診断後経過中ずっと持続し，10年過ぎになると相対危険率RR 1.50(95%信頼区間CI 1.41-1.60)と有意に高くなるのに対して，抑うつは診断後からRR 2.19(95%CI 1.71-2.79)と有意に高いが，それは2年までしか続かず，10年以上の経過でみると次第に低くなることを報告している。こうした背景もあって，主な身体疾患患者のうち，がん患者はオッズ比2.3倍(95%CI 1.1-4.8)と自殺の危険性が高いことが分かっている²⁵⁾。がん患者の不安や抑うつが治療を必要とするレベルになった場合，その多くは精神医学的な診断基準にあてはめると，適応障害とうつ病に相当し，がん患者に最も多くみられる精神疾患である。その評価，診断および治療は，がんそのものの治療を継続し，患者のQOL(quality of life)を良好に保つためにも重要である。そこで本稿では，がん患者でよく問題となる抑うつ状態すなわち，うつ病と適応障害について，その特徴を概説する。
