血液悪性疾患サバイバーの現状と課題—特に造血幹細胞移植に焦点を当てて

はじめに
　東京大学医学部附属病院心療内科では，1996年6月から当院血液腫瘍内科で造血幹細胞移植を受ける患者全例のサポートを行っており，1996年6月から2018年2月までに当科が担当した血液悪性腫瘍患者数は延べ512名であった。当科の医師は，移植前から退院もしくは治療中断(死亡例を含む)となるまでの数か月を，一人の患者と，往診する形でかかわっていく。さらに，患者が希望する場合には，退院後も継続的に外来加療を行っている。
　造血幹細胞移植は，治癒が望める治療である一方，退院後も感染防止のためにさまざまな生活上の留意点が存在する。実際，移植後患者用の分厚い生活指導要綱を手にした途端に，移植の成功への喜びが，待ち受ける退院後の生活への落胆に変化した患者も経験した。
　本邦で最初の造血幹細胞移植が施行された1974年以降，移植件数は右肩上がりに増加し，現在では年間5,000件以上の造血細胞移植が行われている¹⁵⁾。造血幹細胞移植後に報告される症状および症候群には，性機能障害，認知障害，疲労，不眠症，筋骨格症状，感情苦痛，怒りおよび抑うつなどが含まれており²⁰⁾，移植後長期生存者が増える中，医療者は，身体的に健康であるか否かだけではなく，移植後の患者のquality of life(QOL)全体に目を向ける必要がある(表)。
