糖尿病領域との協働

精神科領域における糖尿病治療の重要性
　糖尿病は，統合失調症患者のおよそ13%²⁶⁾，大うつ病性障害患者のおよそ9%²⁵⁾が併発するなど，精神疾患との併存率が高い，精神科治療の場においてもしばしば遭遇する疾患の1つである。糖尿病治療薬の使用によって抑うつ症状や認知機能の改善が認められるなど¹⁵⁾，糖尿病の病態が精神症状にも影響を及ぼし得ることが明らかとなっている。このことから，糖尿病は精神科医療においても無視することのできない疾患であり，患者が糖尿病を併発している場合には，糖尿病の適切なマネジメントが求められる。
　ただし，糖尿病と精神疾患を併発する患者においては，精神症状が改善されるだけでは，必ずしも良好な血糖コントロールがもたらされるわけではない。抑うつを例にとると，薬物療法や精神療法によって糖尿病患者の抑うつが改善されたとしても，血糖コントロールの著明な改善には至らないこと⁴⁾，また治療によって抑うつ症状と血糖の双方が改善されたとしても，それら2つの改善は相互に関連しないこと³⁾が示されている。すなわち，精神科医療の場において糖尿病のマネジメントを考える上では，「精神症状の改善を図ることで血糖コントロールが改善することを期待する」だけでは不十分であり，糖尿病に対する直接的な介入を行うことが求められる。
