緊張病症状にホルモン補充療法が有効であった統合失調症の1例

抄録　統合失調症の発症や経過においてエストロゲンは保護的に作用し，閉経などによりエストロゲンが低下すると症状が増悪することがある。症例は53歳女性，統合失調症。27歳時に幻覚妄想状態で発症し，37歳時より加療開始されたが，44歳時に服薬通院を自己中断した。約9年間の無治療期間の後に緊張病性興奮を呈し，当科入院後に亜昏迷状態に陥った。Olanzapineを開始して一度亜昏迷を脱したものの，以後約1か月ごとに緊張病症状を呈し1週間程度で自然軽快することを繰り返した。ホルモン補充療法を開始したところ緊張病症状は消褪し，約半年後にホルモン補充療法を終了したが寛解を維持した。統合失調症の緊張病症状に対するホルモン補充療法の有効性について考察を交えて報告する。
