双極Ⅱ型障害と不安症の併存

双極Ⅱ型障害は，DSM-Ⅳ3)により診断基準が示され，双極Ⅰ型障害との主な臨床症状的相違点は軽躁病エピソードと躁病エピソードの出現とされている。臨床遺伝学的研究からは，双極Ⅱ型障害患者は双極Ⅱ型障害の遺伝負因が双極Ⅰ型障害のそれよりも高いことが指摘されている9)。また，病相は軽躁病エピソードにとどまるものの慢性の経過をたどり，病相回数は多く12)，さらに，対人関係や社会生活上の困難は多く1)，他の精神疾患の併存も多く14)，自殺の危険が高いことも指摘されている17)。双極Ⅰ型障害への移行は5年で5%程度11)，10年で7.5%程度7)と報告されており，比較的診断の安定度は高いとされる。
併存精神疾患の中でも不安症の併存率が高いことが指摘されており16)，双極Ⅱ型障害患者について不安症の併存の有無による神経認知機能の違い19)や生物学的背景の違い18)，QOLなど2)が検討されてきている。
双極Ⅱ型障害の治療については，双極Ⅰ型障害を参考に行われると考えられるが，どの気分安定薬や非定型抗精神病薬を使用するか，不安症を併存している場合にセロトニン再取り込み阻害薬(selective serotonin reuptake inhibitor：SSRI)などの抗うつ薬を使用するかなど現時点では試行錯誤になると考えられる。
今回，双極Ⅱ型障害と不安症の併存について，その疫学や併存の有無による相違，現在考え得る治療的対応などについて概観してみたい。
