睡眠薬は是か非か　Pros and Cons—Prosの立場から—ベンゾジアゼピン受容体作動薬の是非を問う

はじめに
　睡眠薬は是か非か，という問いに対して「是」の立場で意見を述べるにあたり，議論を散漫にしないためにも，睡眠衛生の指導などの非薬物療法を優先すべきである，といった自明の問題については論点から外すこととしたい。やむを得ず薬物療法を選択する前に，そのようなプロセスを経ることは当然の前提条件であり，その上でなお不眠が解消しない場合に用いる睡眠薬についての是非を論じるのが本稿の役割と考えるからである。
　さらに，論点をベンゾジアゼピン受容体作動薬(以下，BZ)の是非に絞ることもお許しいただきたい。不眠症治療に用いられる薬剤全般について述べようとすると，sedativeな抗うつ薬や抗精神病薬など，不眠症に対して用いた場合の安全性や有効性についての知見が乏しく，保険適用外使用になる薬剤についても検討しなければならないし，BZ以外の睡眠薬，すなわちメラトニン系やオレキシン系の薬剤については上市から日が浅く，長期予後も含めてBZと同列に論じられるほどの臨床経験が蓄積していないと考えるからである。そして何よりも，依存性をはじめとしてさまざまな批判を浴びつつも，BZはいまだに睡眠薬の主流であり，それを完全に凌駕するような新たな薬剤カテゴリーは存在しないという，多くの人が目を背けている事実がある。睡眠薬にまつわる社会的な関心や誤解の多くもBZに対するものであり，BZに絞って是の立場で意見を述べることにより，睡眠薬全般についての理解を深め，適切な睡眠薬治療が行われるための一助となるよう，議論を進めていきたい。
