双極性障害における病識・疾病認識

抄録　本論文では，病識を持ちにくくする双極性障害に関する特性，それ以外の要因などについて，これまでの知見を整理した。病識は治療の経過や症状である攻撃性に影響するが，一方病識を回避することには，セルフスティグマに対する保護という側面があり，個々の当事者の病識に関するきめ細かい理解が重要である。この理解の一助とするために，本論文では，「躁状態・軽躁状態・うつ状態への病識を持つことの困難」「家族との関係への影響」「セルフスティグマ」「治療や病気が生活や人生に及ぼす影響のコントロールの方法を身につけることが，病識を伸ばす上で重要であること」などに関する当事者の見方を紹介した。なるべく早い段階で病識の獲得へのアプローチを行うべきであることは，当事者も指摘している。その人なりに病識を得ていく過程を伸ばすような方向で，サポーティブに手助けすることが，医師に求められていると言えよう。
