展望

統合失調症の認知行動療法

抄録　統合失調症は，難治の陽性症状や病識の持ちにくさや日常生活の障害があるが，認知行動療法はその中で，対人関係，症状対処，服薬自己管理などのツールとして用いられる。認知行動療法は学習心理学に基づき新たな行動の学習を促進する理論から出発して，認知科学の発展により，送信技能だけでなく受信・処理技能も介入の焦点とする，認知的な介入を含む理論や技法へと発展した。精神医療が地域生活支援に移行し，本人の主体性を重視するようになったことと平行して，認知行動療法も，学んだことを当事者自らが使おうとする試みなど，学習の般化への主体の影響が重視されるようになっている。その流れの中で，empowered SSTが提案されている。またうつ病の認知療法をもとにして，精神病体験への認知療法も発展した。エビデンスに基づく治療ガイドラインでも，生活する場での援助を含むスキルトレーニングや残存する精神症状に対処する認知療法が推奨されている。
