PTSD関連疾患—DSM-5との比較を軸に

抄録　ICD-11では，PTSDの中核症状は3つとなり，新たな疾患としてcomplex PTSDが示されることとなった。これにより，ICD-11の診断分類はDSM-5とは大きく異なっている。ICD-11では，単回性か長期反復性かという心的外傷的出来事による分類をとらず，症状プロフィールによってPTSDとcomplex PTSDに分類することが大きな特徴である。一方で，DSM-5では解離症状に関する特定用語が採用されたが，ICD-11では採用されていない。急性ストレス反応については，疾患名としてではなく，正常域であるが臨床的に留意が必要な反応との分類がなされた。PTSD関連疾患の臨床に携わる者にとっては，ICD-11で示されたcomplex PTSDの登場が治療技法の発展や患者-治療者関係の維持に寄与するのではないかという期待感もあり，今後が注目される。
