早期介入・早期支援の視点から—統合失調症の治療ゴールの検討

抄録　本稿では，はじめに初回エピソード精神病(first episode psychosis：FEP)に対する早期介入サービスの成果と現在の課題について論じ，その後，FEPの治療における抗精神病薬の役割について最近の研究を紹介し，治療ゴールの視点から検討を加えた。さらに，精神病に移行するリスクの高い臨床的ハイリスク(clinical high risk：CHR)状態について，精神病への移行を過度に重視することの問題点とその治療について論じた。最後に，早期介入にかかわる医療スタッフからみた，統合失調症の治療ゴールについて意見を紹介した。精神疾患の早期段階からの経過は多様性が高く，その後の治療や環境の影響を受けて変化する余地は大きい。古典的な医学モデルに基づく信念や偏見によって，当事者の治療ゴールの可能性を狭めるべきではない。臨床的リカバリーとパーソナルリカバリーを統合的に実現していくための支援を早期から行う実践とそのための新たな研究に期待したい。
