双極性障害概念の展開と心因

抄録　20世紀初頭にKraepelinが広大な躁うつ病概念を確立した後，Bleuler，Schneiderによって躁うつ病概念は狭小化されることになった。ドイツ精神医学の影響を強く受けたわが国でも躁うつ病概念は非常に狭いものであった。診断概念の統一化を目指す動向の中で，内因性精神疾患の治療反応性・転帰を決定するものは統合失調症症状ではなく気分症状であることが示された。この見解は最初の本格的な操作的診断基準であるDSM-Ⅲに全面的に取り入れられ，躁うつ病概念は重症極へと拡大された。躁うつ病は，その関心の高まりとともに双極性障害と呼称を変え，Akiskal，Ghaemiらによりその双極スペクトラム概念を通して軽症極へも拡大された。その一方，双極性障害の過剰診断の問題も看過できないのが現状である。純粋な躁病やうつ病はむしろ特殊な表現型であり，躁うつ混合状態こそが躁うつ病の本質であることを看破していたKraepelinの卓見は今日なおもその妥当性を失っていない。内因-心因をめぐる議論は治療への貢献を目的として行われるべきものであり，死別後の躁病，“葬式躁病”は，治療を前提として内因-心因問題を論じるには最適の領域であることを指摘した。
