一般病院における高齢者のせん妄マネジメント

抄録　一般病院に入院する高齢者が増加している現状の中で，せん妄マネジメントは喫緊の課題である。せん妄はいったん発症すると中長期的な予後を悪化させることが明らかとなってきており，予防的介入が重要である。入院後速やかにせん妄リスクの評価を行い，リスクの高い患者を同定して，非薬物療法的介入を開始するとともに，一部の患者では予防的薬物療法も選択肢となる。適切なスクリーニングツールを活用してせん妄の早期発見に努め，直接因子の同定とその改善につとめながら，リスクとベネフィットを十分考慮して薬物療法を行い，せん妄の改善をめざす。リスク評価，予防的介入，早期介入とシームレスにせん妄マネジメントを行うためには，施設あるいは部署ごとに組織的に対策を考える必要がある。その実現のためには，リエゾン精神科医および精神科リエゾンチームが中心的な役割を果たすべきである。
