高齢者のがんと精神科急性期医療

抄録　地域包括ケアの支援体制の整備が進む中で，高齢がん患者も外来治療にシフトし，可能な限り地域で過ごすことが多くなった。終末期においても，なお本人の療養生活の質を高めるための取り組みとしてアドバンス・ケア・プランニングがある。わが国では，アドバンス・ケア・プランニングを，万が一の時に備えた代理人を指名する動きのように誤解されがちである。しかしアドバンス・ケア・プランニングは，自己決定権を尊重することを目的にした法制度を含めた取り組みである。また，わが国では障害者権利条約の批准にあわせて，障害者支援が代行決定から意思決定支援に移行しつつある。精神科急性期医療においては，この流れを踏まえた対応が求められる。
