精神科救急における意思決定支援と共同意思決定(SDM)

抄録　精神科救急では，非自発入院や薬物療法の導入，行動制限などの重要な治療上の意思決定場面に多く遭遇するが，当事者の多くが幻覚妄想などの激しい精神病症状，精神運動興奮といった症状を有すること，迅速な対応を要する場面であることなどから，意思決定支援やSDMは行われにくい現状がある。本稿では，海外の先行研究ならびに筆者らが施行した救急入院料病棟における無作為化比較試験(統合失調症初回入院患者におけるSDMの実施可能性)の成果を概観しつつ，精神科救急における意思決定支援・SDMの実施可能性や課題について考えたい。
