Cry of Pain—自殺の認知臨床心理学

抄録　自殺のCry of Painモデルは，希死念慮や自殺企図が生じる過程に認知バイアスを想定した，認知臨床心理学に基づくモデルである。自殺企図者には自伝的記憶の概括化(OGM)がみられることが明らかとなっている。OGMとは，特定の日時・場所で起こったエピソードの想起が困難となり，いくつかの出来事が集約された形で想起がなされる現象である。OGMは，社会的な問題解決の困難や，将来への想像力の低下を引き起こし，それによって適切なソーシャルサポートを受けられないこと，将来への絶望感を抱くことが希死念慮や自殺企図を引き起こすと考えられている。本稿ではこれらの研究をレビューし，今後の精神科医療におけるモデルの活用へ向けた提言を行った。
