注意欠如・多動症(ADHD)と若年性認知症の鑑別

抄録　近年，成人期の発達障害への注目，診断基準の変更，児童期から成人期にかけての臨床症状の経年的変化，老年期の神経発達障害の存在や臨床的特徴についても注目が高まっており，診断の幅が広がった。また，発達障害と認知症の症候学的類似性が指摘されるようになっている。そして，高齢期の発達障害の存在が医学的にも注目される中，若年性認知症との鑑別が臨床上の問題になりつつある。この背景には神経発達障害は主として児童精神科医が，また認知症は老年精神科医が診療に携わっており，成人期から若年期ではお互いがあまり専門としない領域の鑑別が求められ，その両者の視点を持った診療体制が構築しにくいことも影響していると思われる。本稿では，神経発達障害の中でADHDを取り上げ，若年期認知症との鑑別診断の必要性と意義について述べる。
