行動障害型前頭側頭型認知症と自閉スペクトラム症との鑑別

抄録　行動障害型前頭側頭型認知症(behavioral-variant frontotemporal dementia：bvFTD)は，多くが65歳未満で発症し，人格変化や行動障害を主徴とし，前頭葉および/または前部側頭葉に著明な萎縮を呈する神経変性疾患である。自閉スペクトラム症(autism spectrum disorder：ASD)は先天性の疾患で発達期に発症し，社会性と行動の特徴から診断される神経発達症である。後天性の疾患であるbvFTDと先天性の病態であるASDの鑑別は，発症時期や背景病理が異なるため一見容易に思えるが，両者とも共感性の欠如や社会性の乏しさ，こだわりや常同行動が主症状で重なる症候が多いため，臨床現場では未診断の成人ASD患者がbvFTDを疑われて認知症専門医療機関を受診することが少なくない。本稿ではbvFTDとASDの類似点や相違点，鑑別のポイントに焦点を当てて概説した。
