発達障害と認知症を鑑別するための臨床的特徴

抄録　発達障害はその特性が生涯続くものであることから，ロングライフ・ディスオーダーとして捉えるべきであるという考え方が提唱されている。このことは，青年期や成人期だけでなく壮年期や老年期においても，発達特性により顕在化した問題に対し積極的に評価をした上で，治療や支援をしていくことが重要であることを示唆している。このような背景の中，認知症専門外来を受診する患者の中に一定の割合で発達障害を疑うケースが存在することも報告されてきている。そこで，本稿では主に自閉スペクトラム症(autism spectrum disorder：ASD)と行動異常型前頭側頭型認知症(behavioral variant frontotemporal dementia：bvFTD)を中心に症例を提示しながら両者の鑑別について概説を試みる。
