病名告知—難治がんの時代における変遷

抄録　がん治療や早期発見技術の進歩により，年齢調整率でみるがん死亡は減少し，課題は診断・治療から症状緩和，サバイバーシップを含む広範囲に及んでいる。この時代の変遷の中で，がんに関する告知の課題は病名告知の是非から，早期から難治がんまでどのように病状説明を行うか(進行がん，積極的抗がん治療中止，緩和ケアへの移行，予後など)に変わってきたと言える。これらの話し合いは患者・家族，医療者ともに心身の負担を伴うものであるが，患者の意向に即した治療・療養を実現するため，医療者-患者間のコミュニケーションに関する調査，介入研究が行われてきた。患者・家族の意向が反映された治療・療養に繋がる取り組みが全国的にも行われ，患者・家族の負担・苦痛が軽減されていくことが望まれる。
