エジンバラ産後うつ病自己評価票を用いた産婦健診からの妊産婦のメンタルヘルス支援—名古屋大学医学部附属病院における取り組み

抄録　周産期は，うつ病をはじめとする精神疾患の発症や再燃・再発が高率に認められ，妊産婦の生活の質や自死，さらには子への虐待リスクとも関連することから，早期発見，早期介入が求められる。2017年より産婦健康診査事業においてエジンバラ産後うつ病自己評価票(EPDS)が導入された。我々はすでに産科と精神科が協働する体制を構築していたが，産婦健診にEPDSが導入されるのを契機として，妊娠中の外来でのかかわりから始まり，病棟でのケア，産後の外来でのフォローという流れを作りながら，その体制を拡充することとした。加えて，診療報酬でも要請されている地域ケア会議を開催し，地域との連携に努めている。本稿では，名古屋大学医学部附属病院における実践を紹介したが，母子保健と精神保健の連携のためには，さらなる連携実績の蓄積が必要となることを述べた。
