産後うつ病の新規治療薬brexanolone

抄録　産後うつ病は，分娩後2週間以上持続し，日常生活に支障を来す抑うつ症状であり，約10〜15%の女性が産後うつ病を発症する。また選択的セロトニン再取り込み阻害薬などの既存の抗うつ薬では十分な効果が得られない患者も多い。多くの研究から，産後うつ病の発症には，GABA受容体サブタイプAのアロステリック調節因子である内因性神経ステロイドallopregnoloneの異常が関与していることが報告されている。2019年3月に米国食品医薬品局は，allopregnoloneの静脈注射剤brexanoloneを産後うつ病の治療薬として承認した。本稿では，うつ病の病因におけるallopregnoloneの役割，静脈注射剤brexanolone，および現在，第三相臨床試験中の新規経口治療薬SAG-217について述べる。
