統合失調症に対するオープンダイアローグ—メタ倫理としての「不確実性の耐性」

抄録　本稿では統合失調症に対するケアの手法としての「オープンダイアローグ(以下OD)」について，事例をもとに解説を加えた。ODは現在，薬物や入院に依存しない治療方法として，あるいは治療に限定されない「対話の思想」として学際的な関心を集めている。事例は夫に対する被害関係妄想を主訴とした40歳台の女性で，4人の治療チームが本人と夫の患者チームと対話セッションを繰り返した。全10回の対話実践の過程で妄想をはじめとする精神症状が顕著に改善し，薬物治療なしに回復するに至った。本事例では，ていねいな傾聴と応答によって困りごとの言語化を促し，治療者の専門性は脇において，常に患者の主体性とニーズを尊重する姿勢を維持した。その際筆者らは，治療計画よりも治療過程における「不確実性」を優先し，治療チームも対話の過程の一部として振る舞うことを目指していた。
