身体症状症(疼痛)

抄録　身体症状症について，その病態や治療をどのように考え，どのように伝えるのかについてまとめた。まず，最初に身体症状症の診断を考える上で，参考となる架空症例を挙げ，その臨床情報を変更することで見方がどのように変化し得るかを検討した。次に身体症状症から派生する問題を取り上げ，その本質がどこにあるのかについてCOVID-19との関係から考察した。また，身体症状症患者に病気や治療を伝えるときにその障壁となり得る要素についてはアレキシサイミアや認知機能障害を例に挙げた。さらに，治療の基本的な考え方としては，検査や治療などに一定の制限が必要であったり，痛み以外の症状に着目することの重要性を強調した。中心テーマである伝え方の工夫については図式化など筆者の経験を交えながら解説し，最後に共同意思決定をどのように行うべきかについても言及した。
