うつ病への反復経頭蓋磁気刺激(rTMS)治療

抄録　本稿では，2019年6月に保険診療化されたうつ病におけるrTMS治療の適応患者と実施法について概説し，rTMS治療のエビデンス，rTMS治療と電気けいれん療法の対比，国内外の治療ガイドラインでのrTMS治療の取り扱いについて言及した。薬物治療抵抗性うつ病患者におけるrTMS治療の抗うつ効果はすでに頑健なエビデンスがある一方で，うつ病診療におけるrTMS治療の位置付けについてはコンセンサスに至っていない。今後はrTMS治療を適切に普及して診療データを集積し，日本人におけるrTMS治療の効果と安全性を明確にしていく必要がある。さらに良好な治療効果を予測する臨床的知見や生物学的指標が明らかになり，rTMS治療の有効活用がうつ病診療の発展に寄与することを期待したい。
