うつ病への磁気けいれん療法(MST)

抄録　磁気けいれん療法(MST)は，反復経頭蓋磁気刺激(rTMS)を応用した新しい治療技術である。電気けいれん療法(ECT)とMSTは，どちらもけいれん療法であり，静脈麻酔薬，筋弛緩薬などの前処置が必要である。現在までに複数の臨床試験，メタ解析が実施され，MSTとECTの有効性および認知機能の比較検証が行われてきた。概して，抗うつ効果は同等であり，MSTでは発作後の回復が早く，認知機能障害が少ないとされる。これらの知見は，ECTと比較し，MSTのけいれん発作がより限局した領域に作用するためと考えられる。一方，MSTの刺激頻度，刺激時間，刺激回数などの標準化は，今後の課題である。MSTの国内導入にあたっては，日本人を対象とした有効性，安全性の検証を行った上で導入されるべきである。
