うつ病への電気けいれん療法(ECT)

抄録　電気けいれん療法(ECT)は，単極性および双極性うつ病の治療における重要なオプションである。切迫した自殺の危険や身体的衰弱，精神病性や緊張病性の特徴を有する患者では1次治療としての適応になり得る。薬物療法に反応しない，または副作用のために十分な薬物療法を行えない患者では，2次治療としての適応を考慮すべきである。施行にあたっては，既往歴，身体的併存症，各種検査など適切な術前評価を行い，麻酔科医による評価と合わせて可否を判断し，インフォームド・コンセントを行って決定する。発作誘発への影響を考慮して薬剤調整を行い，患者ごとに電極配置や刺激電気量を設定して，誘発された発作の有効性を毎回評価しながらECTを継続する。必要に応じて刺激パラメーターの変更や増強法を併用して寛解をめざす。改善後に薬物療法による再燃再発予防ができない患者では，継続維持ECTを積極的に検討する。
