解離症とサイコーシス

抄録　本稿では，解離と精神病の関係について検討する。まずヒステリー，とりわけヒステリー性精神病と統合失調症の歴史的流れについて概観した。当初，ヒステリーはその派手な振る舞いや症候学，催眠による治療などに関心が向けられ，要因となるトラウマについて取り上げられることは少なかった。20世紀に入ると，ヒステリーと統合失調症がともに「分裂」という言葉によって表象されることにも窺えるように，ヒステリー性精神病は拡大化した統合失調症へと分類され，ヒステリーへの関心は低下した。しかし1960年以降トラウマへの関心の高まりを背景に，ふたたびヒステリー性精神病や反応性精神病が注目され，さらには神経症と精神病の境界の変容から，あらためて解離と統合失調症の関係が問題圏として浮上することになった。最後に解離症と統合失調症の鑑別困難な症例の診断に際しては，主に他者の先行性やまとまりのない発話に注意することが重要であることを指摘した。
