神経発達症における強迫

抄録　強迫症状は，複数のディメンジョンに分かれ，多様性を有することが知られているだけではなく，不安から衝動性に至るスペクトラムを形成し，強迫に関連する障害群の中には，自閉スペクトラム症やトゥレット症も位置付けられる。自閉スペクトラム症においては，衝動性の強い強迫性や常同性がみられるが，不安の存在は強迫症状を増悪させる。トゥレット症における強迫は，just rightと表現される衝動性の色彩の強い強迫性が特徴的であるが，思春期に不安症を伴う例も少なくない。神経発達症の強迫の多くは，ドパミン系の病理を反映するが，不安も関与するなど，その病理は多層的である。神経発達症における強迫の理解は，まだ十分とはいえず，神経発達症の病態と強迫症状の詳細との関連を仔細に検討することで，その解明が期待される。
