摂食障害における強迫

抄録　摂食障害の臨床においては，患者の強迫的な振る舞いがしばしば観察される。これまでに行われてきた研究により，摂食障害の強迫を説明し得る興味深い知見が数多く発見されており，摂食障害と強迫症の関係については，古くから関心深い議論が繰り広げられてきた。強迫的な行為には，認知柔軟性と中枢性統合の障害に起因する可能性があり，この認知機能障害は摂食障害と強迫症のいずれの疾患においても認められる。治療については，強迫症に対しても有効性が確立されている認知行動療法は神経性過食症と過食性障害にはきわめて高い治療成績を誇るが，神経性やせ症に関しては効果が限られている。本稿では，最新の知見に触れつつ，両疾患の共通点と相違点について，診断学，神経心理学，そして治療の側面から包括的に考察したい。
