強迫症へのニューロモデュレーション—DBS，rTMS，ECT

抄録　強迫症(OCD)は，ガイドラインでは曝露反応妨害法などの認知行動療法，選択的セロトニン取り込み阻害薬や第二世代抗精神病薬などによる薬物療法が推奨されている。しかし，これらの治療を十分に行っても治療反応に乏しい治療抵抗性OCDの場合，ニューロモデュレーションも治療選択肢となる。OCDに対する有効性が報告されているニューロモデュレーションには，脳深部刺激療法(DBS)，反復経頭蓋磁気刺激療法(rTMS)，電気けいれん療法(ECT)などがある。これらは，それぞれ立ち位置が異なっている。本稿ではこの違いも踏まえながら，OCDへのニューロモデュレーションに関する知見と課題について論じる。
