医師による自殺幇助—現状と課題

抄録　医師による自殺幇助(physician-assisted suicide：PAS)は，意思決定能力を有する患者の自己決定に基づいて，医師が致死薬を処方し，患者が自ら選択したタイミングで服薬したり点滴のストッパーを開けたりして最期を迎える方法である。「自律尊重原則」を重視し，医師が患者を支援して実現する自死の選択であり，世界のごく一部で行われている。PASは安楽死と類縁関係にあるが，この2者では実施のあり方と医師の関与の程度に違いがあり，医師が致死薬を注射して最期を迎えさせる安楽死のほうが，医師の心理的な負担ははるかに大きいといえる。精神疾患患者をPASと安楽死の対象とすべきか否かはいっそう複雑かつ深刻な問題である。オランダとベルギーのようにそれを許容している国もあるが，精神疾患の特徴を踏まえれば，容認されるべきではないという議論もあり，論争が継続している。これは医学的な課題としてよりも人権問題として捉えられるべきであり，精神疾患患者を差別すべきではないという見解もある。
