BPSDに対する薬物治療up to date—J-CATIA 研究とAPAガイドラインからみえたBPSD治療の問題点

抄録　BPSDに対して抗精神病薬などの向精神薬を使用することには「適応外使用」の問題がある。新オレンジプランでは，BPSDに対しては非薬物的介入を優先すべきとしている。実地臨床においてBPSDに対する抗精神病薬のリスクについての検討が必要なことから，大規模前向き研究であるJ-CATIA研究が行われた。この研究の結果から，抗精神病薬をやむを得ずBPSDに使用する場合には，開始直後には頻回の診察が必要であることが判明した。また，APAガイドラインは，J-CATIA研究の結果と齟齬はない。ガイドラインでは，説明を行った上で同意を取得し，少量から開始することも重要としている。しかし，実地の医療と法的なものには大きな乖離があり，今後，BPSDを適応として有する薬剤の登場が切望される。
