超高齢期の不安障害

抄録　不安障害は高齢者のメンタルヘルス上の，最も頻度が高い問題の一つである。高齢者の不安は，心理症状よりも身体愁訴が多く，典型的な不安障害の症候を呈しにくいことが特徴である。超高齢者の不安障害のリスク要因には，女性，うつ病や認知症の併存，親しい者との死別などのライフイベントなどがあり，これらはおおむね高齢者全般のリスク要因と共通している。一方超高齢者では，不安障害の有病率が前期高齢者よりも減少することが報告されている。そこには，超高齢者は元来身体的かつ精神的に健康な人達の多い集団であることや，疫学研究には健康な人達が優先的に参加することなどが影響している可能性がある。超高齢者では認知症が最大の合併症である。超高齢者が急増する本邦において，認知症恐怖への対応は，超高齢者のメンタルヘルスにおける重要な課題である。
