認知症における過眠症状の評価とモニタリング

抄録　認知症では過眠(日中の眠気，昼寝)がしばしば認められる。認知症患者自身が訴えることもあれば，介護者から「1日中眠っている」「昼夜逆転している」といった訴えによって明らかになる場合もある。加齢変化によっても日中の眠気はみられるが，認知症患者は非認知症高齢者と比較しても過眠が多くみられることが示されている。最近の知見では，過眠症状は認知症に併存するだけでなく発症に先行する早期徴候として出現することが示唆されている。過眠症状のモニタリングは認知症の早期診断や予後予測に有用である可能性があるが，認知症患者における過眠は介護負担にはなりにくい場合もあり不眠や不穏などの他の周辺症状よりも見過ごされやすいと考えられる。本稿では認知症と過眠症状に焦点をあて，そのメカニズムとモニタリング方法について概説する。
