抄録　死別は，精神障害の発症または悪化を引き起こす主要なストレス要因の1つである。多くの遺族が通常の悲嘆反応を示し，臨床的介入を必要としないが，一部の遺族はうつ病，適応障害，複雑性悲嘆などを生じ，強い精神心理的苦痛や機能障害が引き起こされる。
　複雑性悲嘆と他の精神疾患は本質的には異なるものと考えられており，複雑性悲嘆は，大うつ病性障害や心的外傷後ストレス障害，適応障害とは独立した疾患概念として診断基準化が検討されてきた。米国精神医学会が定める精神疾患の診断・統計マニュアルと世界保健機関が定める国際疾病分類において悲嘆に関する診断基準が設けられているが，診断基準に含まれる症状や持続期間などについて差異があり，議論は現在も続いている。
　遺族の抑うつや複雑な悲嘆，心的外傷後ストレス障害の評価には診断基準や評価尺度が用いられるため，診断基準を示すとともに本邦でも使用可能な評価尺度について概説した。
